Cortisol is a steroid hormone produced by the adrenal gland. It is released in response to ACTH (adrenocorticotropic hormone), which is produced by the pituitary gland. Cortisol is the main hormone involved in stress and the fight or flight response. A 24hr urinary cortisol will not show diurnal variation, it measures the total amount of unbound cortisol excreted in 24 hours. This test is done along with serum cortisol to confirm that the high cortisol is truly abnormal.
This test requires a 24 hour collection. Empty your bladder completely first thing in the morning and collect urine everytime you go to the toilet for the next 24 hours, including the first time the next morning.
